"In regards to" is slightly informal and wordy. "Regarding" is sharper and more professional for a medical referral.
Use clinical terminology. "High sugar levels" is layman's language; "hyperglycemia" demonstrates professional lexical resource.
Direct commands ("Please monitor") can sound rude to a senior colleague. Use passive voice or polite requests to improve your Genre & Style score.
