In short

  • Realistic 2026 OET case notes across six professions and three practice-complexity levels, each a full scenario with a writing task.
  • Every case shows what an examiner expects you to include and what to leave out, mapped to the six OET criteria.
  • Each case downloads as a free branded PDF; the complete pack is one email away.

The library at a glance

Levels reflect practice complexity — how many distractors and how much clinical density a case carries — not OET tiers. OET is a single B2-level exam; the levels just help you build from clean structure on simple cases to ruthless selection on dense ones.

How to practise with case notes

More practice without feedback rarely lifts a candidate from Grade C to Grade B. Practising the right way matters more than practising more.

1

Read twice

Once for clinical understanding. Once to identify recipient, letter type, and purpose. Spend 5 minutes.

2

Highlight what's relevant

Mark only what the recipient needs to act on. Cross out irrelevant social details, unrelated history, and anything that doesn't affect the recipient's decision.

3

Sketch a four-paragraph structure

Opening (purpose), background (relevant history), current situation, request and closing. Decide which case-note items go in which paragraph before writing.

4

Write in 30 minutes

Aim for 185 to 195 words. Don't pause to perfect a sentence. Editing is faster than drafting.

5

Submit for correction

Self-review only catches surface errors. The patterns blocking Grade B are usually invisible to the writer. Three to five corrected letters typically reveal them.

Turn this into real practice

Write a letter from one of the cases above, paste it into the free checker for an instant read, then submit it for a human grade against all six criteria — line-by-line feedback from Dr Mariam's team, from $12 (24 to 72 hour turnaround).

Case Notes Library FAQ

What are OET case notes?
OET case notes are the structured patient summary you receive in the OET writing sub-test, followed by a writing instruction. They include patient details, presenting complaint, history, examination, investigations and a task — referral, discharge, transfer or advice. Your job is to write a 180 to 200 word letter using only the relevant information for the named recipient.
Are these OET case notes free to download as PDF?
Yes. Every case note in the library downloads as a free branded PDF with no sign-up. The optional complete library pack — all cases in one document — is available in exchange for an email so we can send a few genuinely useful OET writing tips.
Are these official OET case notes?
No. These are realistic 2026-style practice case notes written by Dr Mariam's OET writing team, who have marked over 11,000 letters since 2014. They mirror the real OET format and the six criteria in place since August 2018, but the official samples live on oet.com.
How should I use the case notes library?
Pick a case for your profession, write the letter under timed conditions using only the relevant notes, then check it. Each page shows what an examiner expects you to include and cut, and links to an annotated sample letter so you can compare your selection against a Grade B answer.
Where can I get free OET case notes for practice?
Every case in this library is free to use, and our free AI Case Note Generator produces unlimited profession-specific scenarios for medicine, nursing and pharmacy on demand. Limited official samples also live on oet.com. All mirror the real OET format for letter-writing practice.
Are OET case notes the same for nurses and doctors?
No. OET writing is profession-specific. Nurses receive nursing-handover case notes (transfer, discharge, referral to allied health); doctors receive more diagnostic, treatment-focused medical notes; pharmacists receive medication-focused scenarios. The general structure is consistent, but the clinical content reflects your profession.

Stuck below Grade B on OET case notes? Dr Mariam's expert OET writing correction service marks every letter against all six OET writing criteria and returns line-by-line corrections within 24–72 hours.
